Known by its generic name Valaciclovir, Valtrex is a prodrug meant to be converted in vivo to Aciclovir. This conversion into an active drug happens after ingestion and is meant to bypass the low bioavailability of Aciclovir, a strong viral DNA polymerase inhibitor. It is used for treatment for genital herpes, varicella, and cold sores. It is a prescription drug which has been available in the United States since November 2009.
Coming in 250mg, 500mg, and 1g tablets, Valtrex is usually administered in different dosages depending on its purpose. The common initial dose is 1g twice or thrice daily for a week for genital herpes and its derivatives, similar to that of an antibiotic. On the other hand, 2g twice a day is recommended for cold sores. This drug could be taken with an empty stomach.

Short-Term Side Effects
Quite similar to Aciclovir, Valtrex is said to cause short-term side effects such as headache, nausea, vomiting, and diarrhea, among others. Although rare, there have also been reported cases of vertigo, sore throat, and constipation. If such symptoms persist, it is advisable to consult a doctor.
Long-Term Side Effects
Renal impairment is often cited as one of the negative effects of Valtrex in the long run, although this seems to be more pronounced on patients who already have pre-existing renal illnesses prior to medication. Some other rare adverse effects mentioned in studies include anorexia, fatigue, anaphylaxis, and seizures.
How It Works
Valtrex is first introduced into the body as a passive antiviral drug, which is later converted into the more active Aciclovir. It is also converted into valine, an amino acid. After several other conversions into various forms, the final monophosphate form attaches itself into viral DNA, which consequently leads to chain termination.
Viral enzymes are said to be unable to get rid of this final form, which deems it effective as an antiviral substance. However, there are certain conditions that are highlighted in which the risks of adverse effects are heightened. These include patients with renal and/or hepatic impairment. As such, it is recommended that the dosage be reduced for patients with cirrhosis or those undergoing dialysis.
Drug Interactions
Official test results coming from the manufacturers show that Valtrex really has no known drug-drug or drug-food interactions which are deemed to be significant enough to have an adverse effect. As such, no dosage adjustment is said to be necessary when taking Valtrex with other substances such as antacids, digoxin, cimetidine, probenecid, and diuretics, to name a few.

The generic name of Valtrex is Valaciclovir. An alternate option is Zelitrex. Both are manufactured by GlaxoSmithKline.
